• What is Advance Care Planning?

• The Importance of Conversation

• Interactive advance care planning reflection/discussion group activity

 

Session 4 (Oct. 24):  Making Decisions for the Future

• The role of your Health Care Agent

• Navigational tour of the health care directive

• Next steps: completion of your directive

• Survey #2: self-rating of difficulty thinking about/discussing death and dying
